Can I set up direct deposit for Mutual of Omaha claim payments?

Mutual of Omaha offers an option for direct deposit of claim payments for Short-Term Disability (STD), Long-Term Disability (LTD), and Critical Illness. Most of the time you will have set this up during your initial filing of your claim. If you chose to receive a check instead but want to change to direct deposit please follow these steps to set up direct deposit: 

For Short-Term Disability and Long Term Disability:

  • Download and complete the form below titled "Disability EFT Form"
  • Once you're done, please submit the form along with a voided check, bank deposit slip using one of the following methods.

For: Critical Illness: 

  • Download and complete the form below titled "Critical Illness EFT Form"
  • Once you're done, please submit the form along with a voided check, bank deposit slip using one of the following methods.

    • Fax - 402-997-1835 
    • Mail - United of Omaha Life Insurance Company HO8W-GDMS 3316 Farnam Street Omaha, NE 68172-7420

Generally, it takes approximately seven business days for direct deposit to be activated once received.
