Mutual of Omaha offers an option for direct deposit of claim payments for Short-Term Disability (STD), Long-Term Disability (LTD), and Critical Illness. Most of the time you will have set this up during your initial filing of your claim. If you chose to receive a check instead but want to change to direct deposit please follow these steps to set up direct deposit:
For Short-Term Disability and Long Term Disability:
- Download and complete the form below titled "Disability EFT Form"
- Once you're done, please submit the form along with a voided check, bank deposit slip using one of the following methods.

Generally, it takes approximately seven business days for direct deposit to be activated once received.
