Upon the diagnosis of a critical illness, you must complete and submit the items below. Complete instructions are available on the Critical Illness claim form:

Submitting the Claim Form
You can submit an Accident claim by mail, email or fax. Simply download the form, print, complete and sign. You can fax the form to (402) 997-1835, email it to submitgrpci@mutualofomaha.com or mail to:

Checking your Claim Status
Once you've submitted your claim, the Mutual of Omaha Employee Portal allows you to view current claims.
