Your Mutual of Omaha plans come with the additional benefit of being compensated for certain health screenings you obtain. Please follow the steps below to submit your health screening benefit:
Items Needed to Submit a Claim
If you have completed the requisite health screening, you must complete and submit the items below. Complete instructions are available on the Health Screening Benefit form:
- Health Screening Benefit form: Download the form attached to this article and complete as instructed on the form or
- Complete an online submission of the health screening benefit
- Select I am a "Plan Member (Employee)" and your state from the dropdown
- Select "Health Screening" and click "Complete the Form Online"
- Complete the information on the next page
- Please note: You will need the the STG Family of Companies Mutual of Omaha group number which is G000AZBL
Submitting the Claim Form
If you choose not to complete the health screening benefit online you can submit a health screening claim by mail, email, or fax. Simply download the form, print, complete and sign. You can fax the form to (402) 997-1835, email it to submitgrpacc@mutualofomaha.com or mail to:

Checking your Claim Status
Once you've submitted your claim, the Mutual of Omaha Employee Portal allows you to view current claims.
